The British broadcaster engaged in a "major breach" of media regulations by not revealing that the presenter of a documentary about Gaza was the offspring of a Hamas representative, as determined by British broadcasting authority Ofcom.
An probe by the broadcasting watchdog into the programme Gaza: How to Survive a Warzone has concluded that the film was "substantially inaccurate".
The head of the BBC had already issued an apology, stating there had been "a serious error in relation to truthfulness".
Ofcom has directed the BBC to air a evening declaration about its findings.
"Since this amounts to a serious breach of our rules, we are ordering the corporation to transmit a message of our determinations against it on BBC Two at nine o'clock in the evening, with a date to be finalised," it said.
'Risk to Undermine Credibility'
The regulator's report declared: "The inquiry we conducted found that the documentary's failure to disclose that the voice's father occupied a position in the Hamas-controlled authority was substantially inaccurate.
"It meant that the public did not have vital facts which may have been extremely pertinent to their assessment of the voice and the content he presented.
"Confidence is at the core of the relationship between a broadcaster and its audience, especially for a state-funded media outlet such as the BBC.
"This oversight had the possibility to damage the very considerable levels of trust that viewers would have placed in a BBC factual programme about the Gaza conflict."
The film was removed from the BBC's streaming service in February after the presenter's personal associations emerged, and in July an broadcaster's internal assessment determined it breached the broadcaster's content policies on factual correctness.
A representative for the broadcaster said: "The Ofcom ruling is in accordance with the conclusions of the internal assessment, that there was a serious error in the documentary in relation to the BBC's Editorial Guidelines on accuracy, which aligns with Rule 2.2 of Ofcom's Broadcasting Code.
"We have apologised for this and we comply with the authority's determination in complete. We will comply with the penalty as soon as the schedule and wording are confirmed."
